Scope and source boundary
Human validation is an explicit gate; rejecting a pending request must not trigger the guarded operation.
Human-in-the-loop work keeps proposals and irreversible business effects at different authority levels. The source distinguishes an agent recommendation, a pending confirmation and an authorized execution. Confirmation must identify the intended request, preserve its arguments and prevent duplicate dispatch.
